Where Are You Now?
Receiving a scholarship from the FNSNA can be a life changing experience. The funding provided assisted your nursing education and supported your dedication to achieve the goal of becoming a registered nurse. However, we don't know what's happened since receiving that scholarship.

The FNSNA wants to reconnect with past scholarship recipients like you to answer the question "Where are you now?" The FNSNA will be featuring stories from scholars who reconnected with us in all forms of communication including but not limited to Imprint, www.fnsna.org, and all social media platforms.

I hereby give the Foundation of the National Student Nurses' Association, Inc/National Student Nurses' Association, Inc. (FNSNA/NSNA), and its successors and/or assigns, the absolute and irrevocable right and permission, with respect to the photograph attached to

(a) to copyright same in its own name or any other name that it may choose;

(b) to use, reuse, publish and republish the same in whole or in part, individually or in conjunction with other photographs or images, in any medium, and for any purpose whatsoever, inlcuding but not limited to illustration, promotion, advertising and trade, and publication in all FNSNA/NSNA publications and media of any kind or description, including but not limited to Imprint magazine and the FNSNA/NSNA website; and

(c) to use my name in conjunction therewith if it so chooses.

I hereby release and discharge FNSNA/NSNA, his or its heirs, executors, assigns and any designees (including any agency, client, broadcaster, periodical or other publication) from any and all claims and demands arising out of or in connection with the use of such photographs, film or tape, including but not limited to any claims of defamation or invasion of privacy.
